After you complete By the Gods – Stringing Up Sods quest in Witcher 2 Assassins of Kings Enhanced Edition, you will have the chance to start a side-quest called Troll Trouble.
This specific quest will lead you to an item of extreme importance, required if you want to beat Melitele’s Heart side-quest.
To begin Troll Trouble, you have to check the board near the inn, and pick up a contract, which allows you to activate the side-quest.
Quest Description
In practicing their trade, witchers abide by a code that clearly explains what they can and cannot do. Geralt claims he invented this code himself, to have an excuse for refusing unwanted jobs, but I think such a stratagem would be too complicated for him.
Either way, Geralt noticed that Bernard Loredo had posted a bounty on the head of a troll living in the area. Because the troll is a sapient creature it falls under the code as huntable only if evil. The curious witcher decided to check it out.

Walkthrough
After you activate the quest, visit the small village on the eastern side of Flotsam, and look for a citizen named Chorab. You will find him near a fire in the center of the village, talking about the troll. Talk to him and he will ask you not to kill the troll, but to convince him to stop drinking.
Head east and you should see a large bridge across the river. Unfortunately, the bridge was destroyed. Near the bridge is a villager attacked by nekkers.
Save him quickly then jump down, and under the bridge you will find the troll who asks you if you have some vodka, otherwise he wont let you cross the river.
Since you dont have the booze, tell him that you dont intend to pay to cross a ruined bridge. Eventually, the troll will attack you.
Combat Tip: Heavy attacks and the Igni Sign will help you defeat the creature.
After the trolls health drops to ½, the fight ends, and the creature will surrender. Ask the troll what happened and you will find that the she-troll was killed. Return to Flotsam and talk to Sile, the sorceress, inside the inn (second floor).
Next, talk to Zoltan inside the inn, and travel to Lobinden, where you must find Sendler, the last opponent you have defeated during the Poker Face: Flotsam side-quest. Play a game of dice poker and make sure you win, but instead of Orens, ask him to give you the troll head he has on the wall of his cottage.
Sendler tells you that he purchased the she-troll’s head from a villager called Dimitri. Go back to the inn, in the evening, and look for a character named Feyn in the basement. Try to persuade him to tell you where Dimitri is, or fight him.
After you find Dimitris location (the cemetery), open your map, and east of Lobinden you should see an area marked by three structures (between Lobinden and the eastern river). Travel to this location (at night) and kill Dimitri and his lackeys, then go back to the troll.
First tell the troll that you found the head of his spouse, and then tell him that you killed those who murdered the she-troll. Finally, take the creature out, and collect the troll’s tongue (for the Melitele’s Heart side-quest).
Next, return to Flotsam and talk to Louis Merse for your reward.
