Baltimores Nightmare is a side quest activated while playing the second chapter featured in Witcher 2 Assassins of Kings Enhanced Edition, if you sided with Iorveth.
To start this specific quest, you have to find the white crystal when you explore the old quarry in Hunting Magic.
If you use the white crystal on the projector, you will trigger a short cutscene showing a scared dwarf running on the streets of Vergen. The dwarf is hunted by someone.
Quest Description
“Geralt witnessed various wonders in the harpies nest. Among others, he saw the dream of Baltimore, a dwarf who once lived in Vergen.
Baltimore was now dead, his dream-in which he was running in terror- having become a reality. Before the witcher discover that, however, he would have to find the dwarfs house.”

Walkthrough
Your first goal in this side quest is to find Baltimores house, which is located on the left side of the road that takes you to the Metallurgists Gate. On the right side of the same road is Cecil Burdons house.
Make sure you visit this location during the day, and when you approach the said house, your quest updates.
Near the house, to the left, while facing the door, is an anvil and several dwarves working near it (on top of the stairs). Talk to Thorak about Baltimore, who was their runesmith. Now, return to Baltimores house and explore it. You should find a wall made of bricks which can be destroyed with your Aard Sign.
Behind the wall is a chest containing Baltimores Directions and Baltimores Key. Read the notes, then exit the house and talk to Thorak. You can lie and tell him that you didnt find anything inside the house, or tell him the truth.
Now, head back to the old quarry (you should know the path by now, since you already completed Hunting Magic).
On your way to the quarry you will come across a wooden statue, on the right side of the path. You will see some crows on it. Memorize this location and continue until you reach the base of the quarry; however, dont enter the tunnels where you killed the Harpy Queen.
While facing the entrance, go left and you should see a small hill. On top of it, in the bushes, is a chest. Unlock it to get a Runic Key.
Now, return to the statue/idol you saw earlier and turn left. In front of you will appear a Troll. You should talk to him and ask him what happened, because he will offer additional information regarding the disappearance of Triss Merigold. (Where is Triss Merigold? quest updates after the dialogue, assuming that you didnt play it by now). Make sure you are polite with the troll and after the dialogue, focus on the current quest, and worry about Triss later.
After the conversation with the troll, follow the path to the left, while facing the creature and continue through the canyon and along the mountains edge. Stay on the dirt road and you should find the wreckage of a ship.
Here you should stop and investigate the ship. Observe the highlighted chest and make sure you loot it. Inside is the Journal from Eyla Tarn, which is an important item, required if you wish to complete the side-quest called Mystic River.
Note: Dont exit the area through the green fog because you will die.
After you pick up the journal, look around and you should see a hidden entrance covered by green vines. The entrance is in one of the mountains walls. If you face the ship, it should be behind you. When you enter the secret room, a cutscene will start and you will have to make a decision.
Thorak followed you and he wishes to make a deal with you. You can keep everything inside Baltimores chest, if you give him the notes. Or, you can kill the dwarves and keep the treasures and the notes.
Note: It is a good idea to take them out, and loot their corpses, especially Thoraks because he carries an important key which will help you to finish the side-quest called Suspect: Thorak.
The quest ends as soon as you agree/fight Thorak and his dwarves.
