An Encrypted Manuscript is a Witcher 2 side-quest that can be completed during the third act of the game.
An Encrypted Manuscript can be activated while exploring the ruins near Loc Muinne, (For A Higher Cause! if you allied with Iorveth), or when you play The Gargoyle Contract side-quest.
The reward for this quest is one of the best swords in Witcher 2, a weapon called Caeme.
The quest becomes available while you deactivate the seals for The Gargoyle Contract side-quest.
In each chamber you explore you will find a chest, and one of them contains a manuscript.

“In one of the ruined passages in Loc Muinne, Geralt found a manuscript written in a strange language.
He could not understand a thing of it, so he decided to find a professional who would help him decipher the document.”

Walkthrough
After you get the document talk to Bras of Ban Ard and he will tell you that he needs several rare ingredients to decipher it: the pheromones of an Endrega queen, the blood of a Nekker warrior, the brain of a Bullvore and a harpy egg (or a Rotfiends tongue).
Note: Without these ingredients you cant complete the quest.
If you have them, ask Bras of Ban Ard to decipher the document, which is in fact the diagram for the Caeme sword; but keep in mind that to craft it, you need even more materials and 250 Orens.
Once you get the sword, equip it, to complete the quest.
