Hades Nexus: Prothean Sphere is a side-mission in Mass Effect 3, which can be completed during the second act of the game.
To start the mission, you must visit the Docks: Holding Area in the Citadel.
Once you get there go to Bay E28 and locate a bald refugee.
If you approach him the side-quest Hades Nexus: Prothean Sphere will be added to your journal.

“A refugee suggested that a Prothean sphere could be located somewhere on Gei Hinnom in the Hades Nexus.
Locate the sphere and inform the man at the Citadel refugee camp.”

Walkthrough
To complete Hades Nexus: Prothean Sphere, go to Normandy and open the Galaxy map in the CIC.
Locate the Hades Nexus Cluster (NE side of the map), and enter the system.
Travel to Sheol and then to Sheol System.
Scan the southern side of the system and you will find a small planet called Gei Hinnom.
Next, scan Gei Hinnom and retrieve the Prothean Sphere.
Take it back to the refugee in Bay E28.
Next side-mission is Hades Nexus: Obelisk of Karza.
