When you reach the Inpatient Wing, turn left and walk inside the examination room.
You will hear a doctor reporting about a general killed with a poisoned blade.

The toxin sample can be retrieved during the Arrae: Ex-Cerberus Scientists side-mission.
Return to the doctor with the toxin to finish the quest.
