Melitele’s Heart is a Witcher 2 Assassins of Kings Enhanced Edition side-quest which starts during the game’s Prologue.
Melitele’s Heart is a choice-based quest, which means that the decisions you take will affect the quests ending.
The following walkthrough explains what options you have while playing Melitele’s Heart quest, as well as their consequences; however keep in mind that Melitele’s Heart ends much later in the game.
Quest Description
“In Foltests Camp, Geralt ran into the Crinfrid Reavers. Having lost his memory, the witcher no longer recalled the trouble this bunch of rogues had caused during the famous hunt for the Golden Dragon. As it turned out, Kennet, Desbrut and Boholt had recruited a new comrade. With great originality, they called him Newboy.
The four were to join in the assault on La Valette Castle that day. The problem was that the drunken Reavers had bet one of Foltests knights that Newboy would survive the battle while attacking solely in his trousers and shirt. A protective amulet they had plundered from a roadside shrine was to help him.”

You begin this quest by talking to the Crinfrid Reavers in the Temerian Camp. According to the dialogue, they know you and you have met them while fighting a Golden Dragon. After that fight, they continued to hunt monsters and they ended in Foltests Army, along with a new mercenary named Newboy.
Seems that Newboy possesses a magic medallion (amulet); however Newboy made a bet that he can survive the assault without wearing armor, and he will be protected by the amulet. You are asked to check the amulet and see if it has any magic properties. At this point you have two options:
Option 1: The amulet could help You will find Newboy dead, later in the game, and you will be able to take his amulet from him.
Option 2: I doubt it Newboy survives and you will meet him later.
If you decide to trick Newboy, and let him die, his corpse can be found on the streets of the city while playing the main-quest known as To the Temple.
If you go with Option 2, you will meet Newboy when you escape the dungeons, during the main quest called The Dungeons of the La Valettes. You will run into him when you exit the prison. He agrees to help you escape, but he may die. Make sure you loot his corpse for the amulet.
In the first act of the game, when you look for Cedric, in the village of Lobinden, there is a specific cottage on the right side of the river, to the north. Inside you will find a woman named Anezka.
Talk to her and tell her about the amulet you have (assuming that you retrieve it from Newboy). At this point, you have two options:
Option 1: Sell the amulet to Anezka, and finish the quest immediately. It is the fastest way and you should pick this only if you already completed the Troll Trouble side-quest.
Option 2: Persuade Anezka to tell you more about the talisman. You will find out that the talisman named Melitele’s Heart is a special item, but it was cursed. To lift the curse, Anezka must perform a ritual and she needs some special items:
The tongue of a troll you will find it while playing the Troll Trouble side-quest.
The eyes of an arachas obtainable after The Rose of Remembrance main-quest, when you meet Iorveth in the forest.
The fetus of an endrega can be found in the woods around Flotsam, but you have to defeat some spiders. You can find the fetus while completing The Endrega Contract side-quest.
The essence of death an item which can be obtained while fighting the wraiths featured in a side-quest named In the Claws of Madness.
Once you get all these items, return to Anezka who will perform the ritual for you, at midnight. Meet her in the forest and when she begins the ritual, you will be asked to protect her from the Wraiths that attack her.
Make sure she is not interrupted, and use the Igni Sign against the Wraiths. When the ritual is complete, talk to Anezka and get the powerful artifact back, to complete the quest.
